Changes of Voice Characteristics Following Horizontal Partial Laryngectomy

ABSTRACT

Conservation laryngeal surgery is designed to remove the cancer mass completely and to preserve the physiologic functions of the larynx. Especially, as is well known, conservation laryngeal surgery enables to produce relatively normal voice. But, limited objective data are available on voice characteristics of patients following conservation laryngeal surgery. In this study, we have evaluated functional results from 6 patients with laryngeal cancer who had undergone supraglottic laryngectomy and 7 patients with laryngeal cancer who had undergone supracricoid laryngectomy compared to those of normal laryngeal speakers using voice acoustic analysis and aerodynamic study. In the patients with supraglottic laryngectomy, harmonics to noise ratio(HNR) is smaller than that of normal group with statistical significance (p<0.05). In the patients with supracricoid laryngectomy, jitter, shimmer, mean flow rate, subglottic pressure are greater than those of normal group and maximal phonation time, HNR are smaller than those of normal group with statistical significance (p<0.05). In conclusion, supraglottic laryngectomy has relatively little effect on voice while supracricoid laryngectomy has much influence on voice quality in many parameters. But, considering that there is no true vocal cord and permanent tracheostoma, the voice quality after supracricoid laryngectomy is thought to be acceptable.
