Reconstruction of the Tongue with Free Flap after Total Glossectomy

ABSTRACT

Ablative cancer surgery of the tongue, specifically that requiring total glossectomy, causes profound postoperative speech and swallowing deficits. Previously, pectoralis major musculocutaneous flap with pull-through technique was used safely for the reconstruction of such defect but its inadequate inner linining and postoperative functional deficits were challenging problems. With the advent of free flap transfer technique, several free flaps were used for a variety of tongue defect. Radial forearm free flap was revealed a promising reconstructive option but this flap was difficult to reconstruct a complex defect after total glossectomy because of the lack of volume. Bulky flap enough to compensate the base of tongue in the oral phase such as rectus abdominis free flap provides a good functional results without aspiration. Two cases of tongue cancer were reconstructed with rectus abdominis musculocutaneous flap and lateral thigh fasciocutaneous free after total glossectomy. The functional results were satisfactory. The procedures, techniques and results are discussed.
