Inner Ear Autoimmunity in Patients with Inner Ear Diseases

ABSTRACT

Autoimmunity is pathological processes that are either caused by or associated with the action of an immune response against self antigens. Immune-mediated sensorineural hearing loss that reponds to immunosuppressive therapy let us to search a diagnostic immune study to identify inner ear autoantibodies in patients with autoimmune inner ear diseases. Fourty-four patients with Meniere's disease, sudden sensorineural hearing loss and slowly progressive sensorineural hearing loss were immunologically assessed using immune parameters such as serum erythrocyte sedimentation rate(ESR), C-reactive protein(CRP), immunoglobulins(IgG, IgM), complements(C3, C4), autoantibodies and other studies. Sera were also analyzed by Western blot against fresh bovine inner ear antigen preparation. Of total 44 patients with inner ear diseases, 5 showed antibody against to 68kD, 9 had antibody which reacted with 33-35kD and 16 displayed abnormal responses on immunological screening tests. Six of 16 patients who displayed positive responses on screening tests did not show any band on Western blot. Of 10 patients with Meniere's disease, 3 had antibody against 68kD, 1 had antibody anainst 33-35kD and 6 showed positive responses on immunological screening test. Of 19 patients with sudden sensorineural hearing loss, 7 showed band which reacted with 33-35kD protein, and 7 displayed abnormal responses on screening test. Of 15 ptients with slowly progressive sensorineural hearing loss, 2 showed band which reacted with 68kD, 1 had antibody against 33-35kD, and 3 displayed positive responses in screening test.
