8 Cases of Laryngeal Granuloma Following Endotracheal Intubation

ABSTRACT

Endotracheal intubation has been widely used for general anesthesia or patency of airway. Laryngeal complications such as laryngeal edema, ulceration, infection, granuloma and stenosis may be developed after intubation. Since first Clausen's report in 1932, intubation granuloma has becoming more frequent due to increasing role of intubation in modern surgery. From Mar. 1986 to Feb. 1989, the authors encountered 8 cases of intubation granuloma in out department and following results were obained. 1) Incidence of intubation granuloma during 3 years was 0.046%. 2) Age distribution showed mostly in 20 to 39 years old group(75%). The sex ratio of male to female was 1 : 7, predominantly in female. 3) The location of granuloma was vocal process of arytenoid cartilage in 6 cases and posterior one third of vocal cord in 2 cases. Morphology of mass were pedunculated in 7 cases and sessile in 1 case. The side of lesion was bilateral in 5 cases and unilateral in 3 cases and all in the right side. 4) Main symptoms were hoarseness in all 8 cases, foreign body sensation in 7 cases and dyspnea in 3 cases. 5) Duration of intubation was 1 to 2 hours in 4 cases. 2 to 3 hours in 2 cases, 3 to 4 hours in one case and 72 hours in one case. 6) Duration between endotracheal intubation and symptom development was 20 days to 12 weeks. 7) 7 cases were occurred after surgery such as C/S in 4 cases, thyroidectomy in 2 cases and B/K amputation in 1 case. 1 case was followed prolonged intubation due to aspiration pneumonia. 8) In treatment, granuloma was removed by suspension microlaryngoscopy in 7 cases, which one case recurred, and one case was recovered spontaneously.
