Clinical Study of Foreign Bodies in the Air Passage

ABSTRACT

Aspiration of foreign body is accidentically occurred and most commonly encountered in children, although it is seen in patients of all age. It is not common, but if occurred the diagnosis is almost dependent on history of aspiration, and emergency care is required as soon as possible. 22 cases of foreign body in the air passage were experienced at the Otolaryngology Department of Han Yang University Hospital from October 1981 to April 1988. The results were as following ; 1) Sex distribution was male 3.4 as female 1. 2) Age distribution were 4 cases(18.2%) under 1-year old, 16 cases(72.7%) in 1 to 5 year-old, and 2 cases(9.1%) above 50-year-old. 3) Main symptoms were couging in 15 cases(68.2%), dyspnea in 8 cases(36.4%), and cyanosis in 5 cases(22.7%) in order, and chocking in 4 cases(18.2%), symptomless in 2 cases(9.1%). 4) In physical examination, there were decreased breathing sound in 16 cases(72.7%), wheezing sound in 4 cases(18.2%), substernal retraction in 1 case, and chest pain in 1 case. 5) Chest X-ray finding were as following ; atelectasis in 6 cases(27.3%), pneumonic infiltration in 5 cases(22.7%), and emphysema in 4 cases(18.2%) and normal finding in 5 cases(22.7%), visible foreign body shadow in 2 cases(9.1%). 6) In duration of lodgement, there were most frequent within 1 day in 12 cases(54.6%), 10 days was longest duration and duration was unknown in 1 case. 7) Treatment methods were bronchoscopic removal under general anesthesia in all cases. 8) The natures of foreign body were vegetable in 12 cases(54.6%), all of them were nuts, plastic materials in 7 cases(30.8%), metalic in 2 cases(9.1%), and chicken bone in 1 case.
